AB WELCHER DOSIS VERURSACHT FLUORACETAT VERGIFTUNGSZEICHEN?

Für akut toxische Wirkungen wird eine Dosis von 0.2 mg/kg angegeben. Das entspricht 14 mg Fluoracetat bei einer 70 kg schweren Person.

Für potentiell tödliche Wirkungen wird ein Dosisbereich von 0.5 bis 2.1 mg/kg angegeben. Das entspricht 35 bis 147 mg Fluoracetat bei einer 70 kg schweren Person.

Als Schwellenwert für die sicher tödliche Dosis werden 2 mg/kg angegeben. Das entspricht 140 mg Fluoracetat bei einer 70 kg schweren Person.

Der Fluorgehalt einer Einzeldosis (ED) Moxifloxacin (400 mg) beträgt 19 mg. Levofloxacin enthält pro ED (500 mg) 26.3 mg F. Ciprofloxacin enthält pro ED (500 mg) 28.6 mg F.

Wird also eine Fluorchinolon-Einzeldosis vollständig zu Fluoracetat konvertiert, kommt es laut o. g. Referenzen zu akuter Toxizität.

Bekanntlich persistiert Fluoracetat im Organismus. Folglich könnte es auch im Fall einer wiederholten Einnahme eines nur mäßiggradig metabolisierten Fluorchinolons zu Fluoracetatvergiftungen kommen.

Bei höhergradiger Metabolisierung begünstigt das Kumulationsproblem tödliche Folgerisiken.
Generiert beispielsweise eine Tagesdosis Ciprofloxacin (2x500mg) zu insgesamt 65%
Fluoracetat, besteht laut o. g. Referenzen Lebensgefahr.

KÖNNEN FLUORHALTIGE ARZNEIMITTEL AUFGRUND IHRES FLUORGEHALTS GIFTIG WIRKEN ODER ZU FLUORACETAT VERSTOFFWECHSELT WERDEN?

In der deutschsprachigen Fachliteratur wurde zu Beginn des 20. Jahrhunderts auf schwere systemische Nebenwirkungen durch Anwendung therapeutischer, meist anorganischer Fluorverbindungen hingewiesen. Zur Bösartigkeit und hohen Toxizität fluororganischer Verbindungen und Wirkstoffe gab es in diesem Zeitraum nur wenige, aber ernstzunehmende Hinweise. Mittlerweile ist der internationalen Fachliteratur zu entnehmen, dass fluororganische Arzneimittel zu Fluoracetat verstoffwechselt werden können. Allerdings warnen Fluor- und Medizinalchemiker in diesem Zusammenhang nicht explizit vor dem Risiko systemischer Vergiftungen. Dieser Widerspruch kann mit der Tatsache zusammenhängen, dass zumindest bei monofluorierten Krebsmitteln eine Verstoffwechselung zum Antimetaboliten Fluoracetat beabsichtigt ist.

GIBT ES EINE ÜBERSICHT WELCHE MEDIKAMENTE BETROFFEN SIND, UND WELCHE WIE GIFTIG SIND?

Aus naheliegenden Gründen sind nur für monofluorierte Krebsmittel (Fluorpyrimidine) offizielle Daten verfügbar. Die F-Pyrimidine leiten sich strukturell allesamt von 5-FU ab (5-FU-Prodrugs), d. h. sie verfügen über einen Pyrimidinring mit dem F-Atom an Pos. 5 und werden dementsprechend zu FA konvertiert.

Furtulon, Galocitabin und Emitefur kursieren wohl nur als experimentelle Substanzen. Das hier nicht abgebildete Flucytosin ist strukturell identisch und generiert wohl ebenfalls FA. Es gibt leider weitere ähnlich strukturierte und hochtoxische Wirkstoffe, deren dreckiger Mechanismus bislang nicht gelüftet wurde.

IST DIE ENZYMHEMMUNG DURCH FLUORACETAT REVERSIBEL?

In der Fachliteratur wird übereinstimmend berichtet, dass die Enzymhemmung durch Fluoracetat irreversibel ist. Uneinigkeit besteht darüber, ob das Endprodukt Fluorcitrat das im mitochondrialen Citratzyklus lokalisierte Enzym Aconitase oder Proteine der mitochondrialen Innenmembran (Citratcarrier) irreversibel hemmt. Mittlerweile gehen Experten davon aus, dass beides möglich ist.
Im Übrigen gibt es bislang keine Untersuchungen zu möglichen Interaktionen zwischen Fluoracetat und der Aconitase im Zytosol, obwohl diese analog zur Aconitase im Mitochondrium über ein Eisen-haltiges aktives Zentrum verfügt und somit ein weiteres potentielles Ziel einer Fluoracetat-Vergiftung darstellt.

In experimentellen Studien wurden spezifische Wirkungen der Fluorchinolone auf den bakteriellen Citratzyklus beobachtet. Nach Angaben einer Autorengruppe übte das Fluorchinolon Norfloxacin im Unterschied zu nicht-fluorierten Antibiotika eine spezifische Wirkung auf das Enzym Aconitase aus. Die Autoren beobachteten, dass der bakterizide Effekt von Norfloxacin abgeschwächt werden konnte, indem sie die Aconitase vor der Norfloxacinexposition inaktivierten. Ähnlich gingen Autoren einer Studie vor, in welcher die hohe Toxizität von Fluorcitrat auf den bakteriellen Citratzyklus abgeschwächt werden sollte. Eine rechtzeitige Inaktivierung der Aconitase scheint demnach zu verhindern, dass Fluoracetat als Fluorcitrat an dieses Enzym binden und den Citratzyklus unterbrechen kann.
Zwei in vitro-Untersuchungen belegen, dass Ciprofloxacin im Unterschied zu nicht-fluorierten Antibiotika spezifisch den bakteriellen Citratzyklus beeinflusst. Ciprofloxacin verursachte ausgeprägte Stoffwechselstörungen, die sowohl mit einer signifikanten Herauf- als auch Herabregulierung von Genen des bakteriellen Citratzyklus assoziiert waren.

Im Fall des Schilddrüsenhemmers 3-Fluortyrosin, dessen Toxizität auf einer Konversion zu Fluoracetat beruht, starben 50% aller exponierten Ratten nach einer Dosis von 12.5 mg/kg.

Der Hersteller führte Selbstversuche durch: eine über mehrere Wochen eingenommene Tagesdosis von 1 mg wurde vertragen. Dasselbe gilt für eine über mehrere Tage eingenommene Tagesdosis von 6 mg. Eine einmalige Einzeldosis von 10 mg verursachte wohl keine "erkennbar schädigende Wirkung". Der Hersteller bilanzierte daraufhin:

"Wir raten bezüglich aller 6 mg übersteigenden Einzeldosen noch zur Vorsicht."

Schließlich empfahl der Hersteller bei mehrwöchiger Therapie eine maximale Tagesdosis von 1 mg. Mehr konnte laut Wirkstoffentwickler May "nicht gegeben werden".
Erfahrungen in der Behandlung von Hyperthyreose-und Basedow (DOI)

Der Fluorgehalt dieser Dosis beträgt 0.1 mg. Fluoracetat wirkt jedoch erst ab einer Dosis von 0.2 mg pro Kilogramm akut toxisch. Demnach konnte therapeutisch eingesetztes 3-Fluortyrosin keine schweren Nebenwirkungen verursachen. Der Hersteller kam zu einem ähnlichen Urteil:

"Das von uns erstmalig untersuchte 3-Fluortyrosin nimmt unter allen bisher bekannten Fluorverbindungen insofern eine Sonderstellung ein, als es bereits in einer Größenordnung thrapeutisch wirksam wird, die hinsichtlich des Gehalts an Fluor 0,1 mg pro Tag (für den Menschen) liegt. Bei dieser Dosierung ist natürlich eine allgemeine Wirkung als Zell- und Protoplasmagift unmöglich."

Allerdings kam es selbst bei dieser ausgesprochen niedrigen FA-Dosis nach vier bis zwölf Wochen zu Gewichtsabnahme und Blutzuckeranstieg.

Da beide Symptome mit Fluoracetat-spezifischen Wirkungen assoziiert sind, muss auch bei längerfristiger Exposition im Niedrigdosisbereich von einer Gewebeanreichung des Gifts ausgegangen werden.

Eine durchschnittliche Verabreichungsdauer von 8 Wochen x 1 mg 3-Fluortyrosin/Tag entspricht einer Gesamtdosis von 60 mg 3-FT = 6 mg F. Eine Tagesdosis Ciprofloxacin (2x500 mg) enthält 57 mg F; beinahe das Zehnfache der Gesamtdosis 3-Fluortyrosin nach 8-wöchiger Anwendung.

Beim Krebsmittel 5-FU und weiteren strukturanalogen Fluorpyrimidinen wie z. B. Capecitabin, Tegafur oder Carmofur kann es trotz partieller Fluoridfreisetzung und einer Verabreichung in unterschiedlichen Zeitabständen zu tödlichen oder chronischen Fluoracetatvergiftungen kommen, da
- der Wirkmechanismus der Fluorpyrimidine auf einer nicht-selektiven Tumorhemmung durch Konversion zu Fluoracetat beruht,
- eine Gewebeanreicherung von wiederholt verabreichtem 5-FU dokumentiert ist,
- die Dosisempfehlungen der Hersteller extrem hoch angesetzt wurden (die empfohlene 5-FU-Tageseinzeldosis beträgt durchschnittlich 500 mg/m² KOF (normiert 1.73 m²), was einer 5-FU-Gesamtdosis von 865 mg bei einem Fluorgehalt von 126 mg entspricht -> als unterer Schwellenwert für eine sicher tödliche Fluoracetat-Dosis werden 2 mg/kg angegeben).

Nein. Entscheidend ist die Dosis, der Fluorgehalt, die Position des Fluoratoms und die Metabolisierung des fluorierten Wirkstoffs. Generiert der fluorierte Wirkstoff einen fluorierten Antimetaboliten, der in einen essentiellen Stoffwechselweg eingreift, ist eine potenzierte Toxizität zu erwarten: zum einen konkurriert der fluorierte Antimetabolit mit dem natürlichen Metaboliten, den er imitiert, indem er eine hemmende Wirkung auf dessen Abbauwege ausübt und hierdurch einen Substratmangel verursacht (Beispiel: toxisches 5-Fluortryptophan reduziert essentielles Tryptophan), zum anderen kann dieser fluorierte Antimetabolit schließlich zu Fluoracetat konvertiert werden. Grundsätzlich gilt, dass monofluorierte Verbindungen mit geradzahliger Kohlenstoffkettte ein höheres Toxizitätsrisiko aufweisen, weil sie aufgrund der stabilen Monofluorkohlenstoffverbindung kaum anorganisches Fluorid freisetzen, nachfolgend über den mitochondrialen Fettsäureabbau (β-Oxidation) in C2-Einheiten gespalten werden und somit als Fluoracetat in den Citratzyklus eindringen können.

Leider kann in diesem Fall nur auf Tierstudien verwiesen werden. Vergiftetes Weidevieh hatte ein erhöhtes Vergiftungsrisiko, wenn es parallel zur Fluoracetataufnahme viel Wasser trank. Fluoracetat ist in Wasser sehr gut löslich und wird auf diesem Wege schneller resorbiert. Womöglich korreliert dieser Aspekt auch mit der Giftung fluororganischer Arzneimittel.
Das Vergiftungsrisiko war zudem erhöht, wenn das Weidevieh nach der Fluoracetataufnahme körperlicher Anstrengung ausgesetzt war. Dies hängt vermutlich mit der erhöhten Stoffwechselaktivität zusammen. Es wird am Beispiel einiger Tierarten darauf hingewiesen, dass eine niedrige Stoffwechselaktivität mit einer geringeren Konversion von Fluoracetat zu Fluorcitrat und erhöhten Ausscheidungsraten verbunden ist.

REICHERT SICH FLUORACETAT IM ORGANISMUS AN?

Bisherige Erkenntnisse legen nahe, dass sich subletale Fluoracetatmengen im Organismus anreichern. In Versuchen mit 3-Fluortyrosin und Fluoracetat wurde beobachtet, dass die exponierten Tiere erst nach tagelanger Verabreichung von 3-Fluortyrosin bzw. Fluoracetat mit schwersten Vergiftungszeichen reagierten. Auch nach tödlichen Vergiftungen und Verwesungsprozessen scheint das Gift aktiv zu sein: Kadaver vergifteter Tiere wiesen nach 75 Tagen immer noch toxische Fluoracetatmengen auf. Ältere Daten legen nahe, dass sich Fluorcitrat im Hart- und Weichgewebe anreichert, jedoch ist nach jüngeren Angaben eher im Gewebe angereichertes nicht metabolisiertes Fluoracetat mit persistierender Toxizität assoziiert als dessen Endprodukt Fluorcitrat.
Ein 59-jähriger Kaninchenjäger, der zehn Jahre lang vier Wochen pro Jahr Kaninchenköder mit Fluoracetat präpariert hatte, litt zu Expositionsbeginn unter Gicht und fünf Jahre darauf unter einer Prostatahypertrophie. Obwohl er während des Präparierens Vorsichtsmaßnahmen ergriff und nach eigenem Bekunden kein Fluoracetat aufnahm, mehrten sich im neunten Expositionsjahr Symptome wie Abgeschlagenheit, Erbrechen, Juckreiz, Atemstörungen, Muskelschwund, Herzinsuffizienz, Lebererkrankung, Schilddrüsenunterfunktion, extrapyramidale Störungen und Nierenversagen. Positive Harnproben und eine Nierenbiopsie deuteten auf eine Fluoracetatvergiftung hin.

"3-Fluortyrosin -> Die Versuchstiere waren durchweg Ratten... Die zuerst versuchte Menge von 30 mg/kg erwies sich bei 5 Tieren angewandt auf die Dauer als nicht tragbar. KÖNNEN FLUORACETAT-VERGIFTUNGEN NACH LÄNGEREN LATENZZEITEN AUFTRETEN?

Symptome einer Fluoracetat-Vergiftung können sowohl nach wenigen Stunden als auch nach längeren Latenzzeiten auftreten. Versuchstiere, die ein Fünftel der tödlichen Dosis erhielten, entwickelten offenbar erst nach 48 Stunden schwere Vergiftungserscheinungen. Bei einem Erwachsenen machten sich erste Symptome (allgemeine Schwäche und Parästhesie) trotz tödlicher Dosis erst 14 Tage nach der Exposition bemerkbar; vier Wochen später folgten Bewegungsstörungen, Muskelschwund und Standataxie. Auch wenn solche ausgeprägten Latenzzeiten in bisher verfügbaren Quellen die Ausnahme darstellen, sind sich Experten darin einig, dass die Symptome einer Fluoracetat-Vergiftung aufgrund des komplexen Giftungsprozesses schleichend einsetzen.
